They can be directed against specific target body cells and structures, and to organisms that may enter the body. They can be easily identified by the suffix -mab (an abbreviation for monoclonal antibody) associated with the generic name. The available monoclonal antibody preparations are basiliximab, bevacizumab, certolizumab, cetuximab, denosumab, golimumab, infliximab, adalimumab, tocilizumab and palivizumab. Denosumab is only available in Australia and is indicated in the management of osteoporosis. Please refer to Chapter 64 for a full description of its mechanism of action and clinical considerations. General clinical considerations Monitor for signs of allergy, which include shortness of breath and chest tightness. Emergency equipment, such as adrenaline, H1 antihistamines and corticosteroids, should be easily accessible when using these medicines. Observe the patient for any signs of allergy during treatment, which includes shortness of breath and tightness in the chest. Pretreatment with paracetamol and an antihistamine may help reduce possible hypersensitivity. Tocilizumab is a monoclonal antibody directed towards the interleukin-6 receptor, which is strongly implicated in the pathophysiology of rheumatoid arthritis. Common adverse effects Adverse effects associated with these medicines include an increased susceptibility to infection, rash, headache and gastrointestinal disturbances. Mechanism of action Basiliximab is directed against a type of surface receptor on T lymphocytes. Respiratory distress, serious hypersensitivity and alterations in blood pressure (hypotension or hypertension) may occur during infusion of either infliximab or tocilizumab.

Drugs having such a short half-life are not of much use as anxiolytics, as frequent dosing is required. If the problem is earlymorning awakening, a benzodiazepine with a longer half-life is desirable. Temazepam, which has a half-life of approximately eight hours, is ideal for such problems. Benzodiazepines with longer half-lives tend to produce undesirable subsequent drowsiness. For instance, short-acting benzodiazepines such as triazolam may cause confusion and delirium. For similar reasons, addiction to benzodiazepines is easier to treat when the speci c medicine has a long half-life. It has already been mentioned that di ering chemical structures may a ect receptor subtypes di erently, and receptors for promoting sleep are di erent from those that promote peace and calm. In at least one overdose case with nitrazepam, involving about 50 tablets, the individual did not even fall asleep. Some of the half-lives given are not just for the parent compound but also for active metabolites. In general, the benzodiazepines are a well-tolerated group of drugs with only infrequent adverse e ects apart from drowsiness. Monitor the person for crackles in the lungs, respiratory distress or frothy sputum to determine whether the feed has been aspirated. Determine the positioning of enteric tubes by X-ray prior to administration of feed. Positioning of enteric tubes can be determined only by abdominal X-ray, as these tubes cannot be aspirated. Monitor the gastric tube placement and residual gastric return according to hospital policy or at least every eight hours. Elevate the head of the bed to at least 30 degrees to facilitate movement by gravity. Take the bag out about half an hour before administration and allow to warm to room temperature. If this occurs notify the relevant health professionals, who may order a lower concentration of feed or a slower rate of delivery. Note that timed-release and enteric-coated preparations should not be crushed for administration into a gastric tube. Nutritional assessment should include the cause of weight loss, reason for increased metabolic requirements, and a record of medicines that may have a catabolic e ect. Baseline observations should include bodyweight, temperature, blood pressure, pulse and respiration.
